I think the same thing as in "this is my honest opinion", but I speak rather quickly when I'm in conversation about something intereting/which I'm, passionate about, so "tbh" is a natural way of being more efficient for me.
The orther party in the dialogue generally understands what I mean simply by the context and tone in which I say it.
