I presume you mean DUN error 678. Can be caused by quite a few different multiple of things, bad modem driver, poor line quality. Removed your connection and manually add it back in, make sure to have settings configured, ie go into properties of connection, Networking tab, settings button, uncheck all box in there. You could try reinstalling your modem driver and then droping your Modem speed as well, this normally does the trick. If that all fails your phone line is most likely crap...
If you don't mean the DUN error 678.. well I dunno... ;)

    taken from here: http://www.modemhelp.net/dunerror/error_678.shtml
    1. Is there a handshake? If not the phone number is probably wrong in some way. Are they trying to turn off call waiting the wrong way for their telco? Do they have call waiting? Do they have pulse but are trying to dial tone? Etc...

    2. If there is a handshake check their connectoid and network settings. If all looks good it is probably a handshake error. Make sure the correct modem is selected and setup.

    3. Try upgrading the firmware. To get them on the net in the meantime try lowering the fifo buffers, setting the max speed to 57600, and adding an init string. Try an init that will retain v90 first, but if they just want to get on and download their drivers try setting v34. Uninstalling and reinstalling the modem can sometimes help as well.
they have init strings here too: http://www.modemhelp.net/quickref.shtml

if you have a contextant (HCF, HSF, etc) modem then try bringing the port speed down to 57600 and adding this string in your modem settings:

AT&F&C1&D2+MS=v90,0 (Connection problems, but trying to retain v90)

AT&F&C1&D2&K3+MS=v34 (Force v34) - bring port speed down to 37600 for this one.

check in Control Panel and if you have an option in there that's named 'Modem Helper' open that. there's an option in there somewhere (think it's in modem options) which is called 'Safe Mode 1'. tick that, and try again. this option basically does the same as the string above, but its a bit simpler. Safe Mode 1 turns off V92, and safe Mode 2 turns off V90, but it's likely that it's V92 that causing the problem if you're with Eircom as they don't support V92 afaik.
