Sat theory test - How do I get provisional?

    Hello

    I sat my theory test this morning and passed so happy days. I was told that i could apply for my provisional at the local tax office. What do i need to bring with me to apply? I assume photos etc but do i need to get my eyes tested or anything like that?

    (You don't state which category so I'll assume it's A,A1,B,M or W.

    Completed Application Form D201
    Photos
    Birth Certificate or Passport
    Theroy Test Certificate
    Eyesight Report D502
    Application Fee

    (If it's for a Truck or Bus you'll also require a Medical Report D501)
