unknown modem driver?

hi, i have a modem here that came from some old pc, icant remember where. Now im looking for the driver for it and have no idea where to start? are there any tools i can download to scan for what type it might be? any help? thanks I think its off an old pc-world pc. it has a c-com chipset on it from looking at the card…

I think its off an old pc-world pc. it has a c-com chipset on it from looking at the card itself. its pci but a small pci card as its only has a small few components on it.
