Your local motor tax office has every form you will ever need relating to tests etc. I don't think you can get licence application forms online but you can apply for the test online.
You must apply for the test first and get a letter confirming your application before you can renew your licence though.
Apply for your test online here:

AFAIK your local Garda station will have em.

Clare Co. Council have the driving licence application form to download here:

    http://www.clarecoco.ie/Forms.html#MotorTax

It's not very good quality (basically a bad scan of the form saved as a PDF) but I used it both to renew my Provisional and to apply for my full licence with no trouble (in Ennis).

Some other local authority website might have a better copy, but I haven't found it in the few I've checked.

The councils don't like you using the online forms because it doesn not have the sticky label for your signature that ends up on your licence. I'd advise getting the proper form in your local post office, library or garda station.

I found the driving test application form on the dublin city council website. It's a good quality pdf file http://www.dublincity.ie/Images/drivingtestappform_tcm35-24268.pdf
