How to get a 2nd provisional / learner permit

Hi, My 1st provisional is about to expire and I need to get a 2nd one (I'm assuming I will be getting a learner permit instead of a provisional). My question is what is the procedure? Do I go to the tax office with photos and fill in some form? Got my first licence almost 2 years ago now so I can't remember..... Any advice…

Hi,

Form D201 available at your local MTO. (2nd Learner Permit)
