    Who says it was unauthorised?

Indeed not. Ireland is still a country in which you generally have to do something illegal before you can be imprisoned.

How long would it take to build something like that? Must take weeks to carve it all out. Is the body in cold storage until it's done? Or are they buried, then the headstone is added weeks / months later?

I have no idea how burial logistics work.

In terms of what is possible, one of my siblings is buried in a cemetery in Limerick which has had strict regulations about height and various other details since it opened in the early-80s. So it certainly is possible to legislate for this.

Headstones are generally erected months after burials. The grave, having been opened, has to be allowed to settle before a heavy structure is erected on top of it; otherwise there's a risk that settlement will cause the headstone to shift or fall. And this is a recognised problem if you reopen an existing grave, with a headstone, for a second or subsequent burial; depending on the size and design you may need to remove the headstone and wait for a while before re-erecting it. or you may have to keep an eye on it for a while so that if there is any shifting to you can take steps to realign it or shore it up.

In some parts of the country, superstition holds that if a headstone is erected within the year, the grave will be opened again within the year...
