Doctor Only Card

Hi,

Can anyone please tell me where to find the link for doctor only card on the irish health webpage?? I've tried finding it but cant! I know a pdf document for download is on the webpage as my work collegue told me about it.

Any help would be greatly appreciated!

Its the same application form as the medical card, or it was when I checked it out in June. They will assess you for the medical card and if you do not qualify for that they will then assess you for the GP card.

I know a girl who applied hoping to get a GP card and got a medical card!!!

To apply for a GP Visit Card, you use the same application form as that for a medical card. While your GP Visit Card application is being processed, the HSE will also assess your entitlement for a full medical card.
