I'm local but have a friend in San Fran who was infected with Spyguard.

I suggested he download SmitfraudFix.exe & the instructions on how to use it & it would remove Spyguard.

He has done that but the problem is that SmitfraudFix won't run.

It won't execute at all in any mode.

    Normal mode

    Safe Mode

    User Account

    Admin Account

When he double clicks the file it just doesn't run? (OS is XP)

    Any suggestions?

Did he disable his AV before running it? Some AV's block it.
Is he downloading it from here?

    Edit: Sorry forget that.
    Spyguard is locking it.
    Have a read here

That is the location I gave him to download it from, yes.

I'll have to confirm he has all AV disabled as soon as I can get hold of him (time difference). But I think he has.

Yes, all other security programs are disabled.

Me thinks there's something a little more sinister going on that won't allow SmitfraudFix to execute

I had missed your edit in the earlier post TechieEddy.

He is not getting any mention of Spywarelocked or Spylocked, only Spyguard.

I would think though that something like that is blocking SmitfraudFix from running.

That's what it sounds like. Can he run it in safemode?

Could be a rootkit. Seeing TDSServ around a fair bit.
Theres a guide here on how to remove it. I know it blocks malware bytes so it might also block Smitfraudfix
