Personal PRSA claim

Apologies over the straightforward question but I have been paying into a personal PRSA since Jun 08 and am wondering what is involved in claiming for these payments? Since the budget is due in about 1 hour, I'm looking at everything..:rolleyes: Is it simply ask my PRSA company for a certificate and send it to the revenue?
