Portal (for being the most memorable perspective change - sorry Prey!)
Half-Life 2 (for advancing the art of storytelling in games, + physics)
Resident Evil 4 (for kicking the action genre in the nuts)

PC: Deus Ex - for the level of player choice and excellent level design.
PC: Thief II: The Metal Age - for taking Thief and perfecting it - the level design and story in particular. The greatest stealth 'em up thus far.
PS3: Demon's Souls - for the integration of single player and multiplayer elements into one seamless experience.

Portal - brilliantly designed levels and game mechanic
Hitman - I had Thief 2 down, but then remember how brilliantly organised the missions in Hitman were.
Assassin's Creed - Not the greatest game in the world, but brilliant level design and free running mechanic.
