Hi, was talking to UPC today on the phone. Want to get a free upgrade from 20mb to 30mb. The man said that I need to fill in the request form on the web site. I can't find that form anywhere. Link, anyone? Thanks!

The man said that I need to fill in the request form on the web site.
I can't find that form anywhere. Link, anyone?
