Which health office for changing GP's

Hi all, I got an acceptance letter from my new GP and was told to send it off to my local health office... Does anyone know whether that would be the one in Raheen Industrial Estate, or the one in Catherine Street? citizensinformation.ie says Catherine street, then again HSE website says Raheen... :confused:
