I remember there was a thread on here recently but have done a search and can't seem to locate it.

With regard to the whv extension approved list of postcodes, what if the postcode where you did the work is approved, but the postcode where the ABN of the employer is not approved? And which should be included in the online application form?

Also, are you required to attach any documents (that form 1163 that your employer signs, payslips, etc) with the online application?

    thanks in advance

It's the post code where you did the work that counts, doesn't matter where the employer is reg'd. Most big companies are reg'd in cities
One the form you use the post-code where you worked.
and your employers ABN, there is also a spot for employers address.

As for attaching documents, not at first, you apply and obviously tick the supporting evidence box. then they either grant the visa or request further info. Then you post or scan/upload the forms etc

*Note, if the company is reg'd in a rural postcode, but the work was carried out in a urban area it doesn't count.
