Actually it wasnt a form, I just emailed them at the Contact Us link and got this back a couple of days later, did what they said and I got a kind of a bank statement thing with various random bond #s adding up to a few hundred Euro still in my name, no explanation as to why:

Im going to put 20k in Prize bonds next week once I find my customer number, wages are piling up in my current account doing nothing and I dont trust the markets not to correct soon so not putting it in shares.

PBwinner who started this experiment hasnt exactly been getting the best return but everywhere is offering complete cack rates at the moment so might as well.

Well actually the return hasn't been too bad, when compared to the rates on offer at the banks. I did a quick calculation in one of the past posts in reply to someone who asked about investing in PBs - have a read of that.
