Direct Debit mistake

There has been an unauthorised direct debit that came out of my account for 2 months,only noticed it this month, I contacted the bank and they gave me the orignators reference,it was done electronically by the insurance company that direct debit went to,i contacted the insurance company and they would not give me details…

There has been an unauthorised direct debit that came out of my account for 2 months,only noticed it this month,

I contacted the bank and they gave me the orignators reference,it was done electronically by the insurance company that direct debit went to,i contacted the insurance company and they would not give me details of who actually set up the direct debit but told me to get the bank to cancel it and get it refunded and then to contact the Gardai,the amount is for about €130,

What kind of fraud has happened here
