Eye Test -Learners Permit

Hi, I passed my theory test yesterday so just seeing what paper work and stuff I need to get sorted for my application of the learners permit.
I came across two different kinds of eyesight reports and just wondering does it matter which one you use?

And also I was wondering it says you can get a doctor to sign the form, but how would this be really possible?

The simplest thing is to get your self into an optician. They will have the correct form and be able to complete it.

I printed the D.502 form in 2009 when applying for my LP (and I think it looked more like the first one), but when I went into the opticians, they had the forms anyway so I didn't actually need to print it off in the end.

It's a very common request actually!

Oh I was just wondering like doesnt it say on the form you can bring it to a doctor though, like could you actually do that?
And also how long does it take to get the learners permit? (roughly)
