Help with HTMLHelp Workshop

Have been trying to create help using MS HTMLHelp Workshop and having some problems. Specifically, I cannot get a table of contents or index page displayed. I have, occasionally, been able to add a table of contents file and a file is created by the compiler but it is not displayed by the help viewer. nd absolutely no luck…

I am following the Help within HTMLHelp. I have just ordered manual but I wonder is there something simple I could be missing.

Running in Windows 7 but not sure that should matter and all updates are current.
