AFAIK you can just type up the records yourself, once it's signed it doesn't have to be in the official book. That's only to give you a suggested format. Or you could print off the sheets from the website (link).

Don't quote me on that though...

Or ring the Gaisce office and ask for one.

They are always very helpful there.
