Filing of a defence

Apologies in advance if this was answered already here. What is the form for filing a defence in the Circuit Court. The Circuit Court office told me it is Form 6 but there is no form 6 on the Courts.ie website. I think that the defendant may have to file a defence on affidavit - is this correct?

Affidavits would only be required if Directed or in reply to a Motion seeking Summary Judgement pursuant to Order 28 of the Circuit Court Rules.
