This happens when the DB servers are under high load. We read new posts from the replication slaves while new posts go into master. During high load the slaves get busy and replication slows down, leading to these issues.

It isn't something we can fix except to reduce the high load on the server but optimising out replication lag is next to impossible, so this will happen from time to time. You'll notice it's fixed itself since you reported it, we're currently all systems go
