I was wondering if somebody could tell me what sort of information is on a tax credit certificate.

From Google, I found this image:

    http://tinyurl.com/d78j2wm

Where it says at the top "For the year 1 January 2011 to 31 December 2011 and following years" what does that mean exactly? Does it show all places of employment since 2011?

A tax credit cert is issued to you at the start of every tax year or when you change employments.
What you see on your cert is all your active employments on the date it was issued and how your credits and rate band are split. If you had two active employments in 2011 when the cert issued you would see both on the cert.

The link below will explain a little bit more for you.
