The house needs at a minimum:
Heating installed

Insulation

Rewiring

New windows

Single-floor kitchen extension at the rear, 3.5m x 8m

In addition I'd consider:
Double-height extension at the side, 4m x 8m, extending the existing roof out over this (i.e. no flat roof)

Existing attic conversion with addition of dormer

It's a big job. Ballpark estimates, not including kitchen cupboards, painting, carpets?
