Photoshop favour

I'm trying to photoshop an image just for fun but I can't seem to do what I want to do... I'm just using online photoshop but I thought it'd be ok because in my mind it's quite a simple thing I want done (something to this effect) So, if anyone's half decent with photoshop and has plenty of time to kill could they do this…
