Real world, not per the story. My understanding...

They needed to replace the first Doctor because he was muddling up his lines and was considered too old.

The eight didn't get a chance because they pulled the TV series for 16 years.

The fourth doctor became unbearable and gave the BBC an ultimatum which duly called his bluff.

Thd 9th, 10th and 11th left on their own accord to further their careers.

Is this correct, what about the 2nd, 3rd, 5th, 6th and 7th? Was it simply down to ratings pressure?

Would love to know the exact reasons, such as for the first doctor which are well known.

Can anyone shed light or point in the right direction for a source?

At a guess, they all probably wanted to do other things.

    Start here

Interview with the second doctor, it's in two parts and there are other interviews with him and other past doctors on YouTube also.

Patrick Troughton, Frazer Hines and Wendy Padbury all left due to the long working hours and amount of episodes being filmed.

Jon Pertwee left due to the death of his good friend and co-star Roger Delgado and the departures of co-star Katy Manning and producer Barry Letts.

Peter Davison asked Patrick Troughton how long should he play The Doctor and Patrick Troughton advised him 3 years is long enough.

Coiln Baker was sacked/gotten rid off by BBC1 Controller Michael Grade because of his sacking Baker refused to return to film a regeneration sequence. Grade was good friends with Liza Goddard Bakers ex-wife and allegations that he sacked Baker out of loyalty to Bakers ex wife

Sylvester McCoy was due to do one more year before leaving the show but it was cancelled before he had his final season.

Also there's lots of uncomfirmed rumours the 9th Doctor left because of disagreements with production staff, rather than furthering his career.

Despite CE's comments to the contrary, I definitely believe there's bad blood there given how he wasn't even remotely involved in this special. We'll probably never find out the full story.

As to the reasons for the different actors leaving, I think it's a shame we never got to see Colin Baker flourish. He was really hampered by a lot of things - the BBC strike being a huge one, but also some weak scripts. Anyone who has listened to the audio adventures can get a glimpse of how his Doctor could have been, especially with a companion like Evelyn Smythe.

Speaking at the Doctor Who 50th Celebration at London's ExCel, Moffat said: He has no more ever called himself the 11th Doctor than he would call himself Matt Smith. The Doctor doesn't know off the top of his head what number he is.
That's not true. In one episode he specifically refers to himself as No. 11, not to mention all the references to the "Fall of the 11th".
Paul McGann turns into John Hurt so they're not the same incarnation. He used up another regeneration and I expect he'll be in trouble shortly - you can't break rules laid down in 'The Deadly Assassin'," he teased.
Deadly. So Capaldi is the last incarnation before something special needs to happen to extend the Doctor's regeneration limit. I'm guessing it'll tie in to bringing the Time Lords back.

Sure we can stick another incarnation in between Troughton and Pertwee as that was off screen as well...
