My music teacher claims that for minor melodies (Q1) you can only sharpen the 7th note of a key in the bars that outline the chord of V (bars 12 and 15). Aren't you supposed to sharpen the 7th note no matter where you use this note throughout your melody? Without doing so, wouldn't it not essentially be modulating between two different keys?

You must always sharpen the 7th note so as to indicate your knowledge of being in the minor to the examiner. You only need to show it once in a bar. You may used the flattened seventh if and only if you are indicating a knowledge of the melodic minor (not sure if all schools would teach this)
Eg. If youre in a minor you must always indicate a g # every time you use a G, unless in the melodic.

What should you do if the 7th note is already sharp?

Also don't forget to make sure it's natural before you add a sharp sign to it if it'd a flat note make it natural :)

If it's already sharp then raise it to the next note. No matter if the note is flat, natural or sharp already, you raise the 7th note.
