Appraisal Forms for GP/BST Training

    Hello,

    My question is quite simple really. I'm halfway through my internship (EU student studying in an EU country) and soon I'll be starting the rotations from which I'd like to get appraisal forms to use come application time in November/December.

    I just need a bit of clarification.

    The standard BST appraisal form can be used for both GP and BST (ex. General Medicine) training schemes during the application process, right?

    My plan is to get 2 appraisal forms (since that's what's required according to each faculty's website); 1 from a general medicine rotation I have coming up, and 1 from my internal medicine rotation. Then, use both for the GP and BST training schemes I intend to apply for and if offered an interview, bring them to both sets of interviews (if offered an interview for a GP and BST spot, obviously).

    Anyone gone through this before, please let me know. I'd appreciate your input.
