I thought it was the headgear that the doctor wore that was the issue.

I imagine it was some sort of headgear he was wearing, as foreign doctors often do. S/He may have been Greek Orthodox, Buddhist, Atheist, or a Druid. If s/he looked a bit Eastern, that's enough for people with primitive attitudes
