I'm in a 3 year, level 7 course that allows me to do the 4th year of the level 8 equivalent of my course. Do I have to graduate in the 3rd year even if I'm going to do the 4th year?

Yes, you'll get your certificate etc and will (if you want to go to it) have the graduation.

To get into the 4th year of the course though you'll need to have all exams passed obviously.

Then on completion of 4th year you'll earn your higher degree and will have another graduation.
