Remove name from joint mortgage

Hi, I have a mortgage drawn down in 2006 that my mother came on as co-borrower to support the application. The loan is fully up to date - just wondering: a) is it possible to remove her from the mortgage at this point? And if so, b) what do i need to do? Thanks.
