When I am at home it is usually a ham and cheese toastie...the only variety would be whether I add onions or tomatoes or whether I use english or french mustard.
But at work there is a canteen so have a good selection of options there.

Depends what it is but, no, most of the time I wouldn't bother. If I'm particularly annoyed or the item was expensive I would.

Quite a while. It's not intentional but I generally get embroiled in a battle to get things resolved to my pleasing.
