    I only ever bother installing 'Important' updates but I have a lot of KB ones listed under 'optional' so should I really install all KB updates.

    Right click on each of them and choose Show Details. Should give you an indication about what the update concerns. It's usually compatibility fixes that don't count as Recommended as no security issue is present.
    I'd recommend installing them, they may also improve performance in some cases
