Hi guys sorry for the long post but im lost

To start my screen is cracked and when it with on with the wake/sleep button its ok for a bit then the screen starts to discolour but is fine again for a while after i do the wake/sleep cycle. Pain in the a** i know and makes the process harder.

    *Im running windows 7
    *Itunes 12.2.2.25 64bit

OK so i have an iPhone 5 running ios 8.4 something (i think) i recently upgraded after having ios7 for a long time. After restore i keep getting errors, mainly error 3004.
I keep updating to the latest iTunes and have tried replacing the host files, turning of fire walls etc nothing is working,

At the moment i wake the phone and it looks like a brand new setup as in the Hola/bonjour i click through till the activate screen and it stalls and gets error.
When plugged into iTunes i get the error'' we're sorry we are unable to continue with your activation at this time''.
When i click on fire>devices>sync (etc) its all greyed out.
It does however let me eject the phone

When i put the phone in DFU mode it prompts me to upgrade and restore , i have tried this so many times via downloading in iTunes, using the .exe from itunes/apple, i have also tried using the official .ipw file and holding shift and choosing that manually with trying update/restore.

Whatever way i try i get usually error 3004 or others.

I just cant seem to activate the phone.

Here is the log file from iunes dianogtics
