Is it just me or has the firefox menu bar disappeared completely? I've no File, Bookmarks etc.
When I go to Customise there's no mention of the menu bar in the list of available toolbars.

Press the 'alt' key and it should drop-down for you

Nope, Alt does nothing. Should mention it's a linux system.

Can you right click near the top of the window.

I'm using Peppermint Linux, and FF gives me a "Menu Bar" option that I can check / uncheck.

Well I use various flavours of Linux, and all I ever do is the same as me_irl says above, right click to the right of the last open tab and the option is there to add the menu bar (and the bookmarks toolbar). Did it yesterday after installing Salix and PacBang to Virtual Box.
    I always use them both.
