LOST P-45

my daughter is in the process of applying for a medical card,
however it is on hold at the moment because she has lost her p-45
She is not claiming any benefits and has no income.
the company she worked for last year seem to be unwilling to issue another one
the application cannot proceed because of this,

any advice anyone

Medical card cannot proceed with her application unless they have evidence of her income.
Either she is in full time education and a dependent of yours, or she is in employment, or she is claiming a SW payment.
Have you spoken to medical cards over the phone?

the sticking point here is . is it possible to get a copy of a p-45.
her last employer will not answer.

You can ask but the employer doesn't have to comply. A P45 was already supplied as per procedure.
