What is the procedure for a chargeback ?

Do you have to instruct the bank to do a chargeback ? or can a bank do a chargeback on its own accord ?

Depends on the nature of the issue, some transactions (if not properly authorized) could generate an automatic chargeback, this would tend to apply to "card not present" transactions.

If you want to dispute a transaction with your Card Issuer, you need to call them and discuss the matter directly with them, depending on the circumstances, they might try to dissuade you from raising a chargeback.

Typically you might raise a chargeback for a fraudulent transaction. In certain circumstances, you can also chargeback "genuine" transactions, for example, an online order that did not arrive.
