The top of my hot water cylinder looks like this:

I want to fit a shower pump. For this, do I need to fit a Surrey flange? And does it look like it will slot in ok?

You'll need a flange. Without one you will damage the pump & it won't be covered by the warranty (5 years in the case of some pumps)

Ok - so based on the photo, should it be just a matter of disconnecting the elbow joint and installing the flange between it and the cylinder?
And should I connect the shower hot water feed at the top of the flange?

Unscrew the elbow out of the tank and fit the flange in accordance with the instructions on the packaging.
Depending on which one you buy, the pump connection may be from the side or the top.
Additional pipe connections will be required and maybe some adjusting of existing pipework. Ensure the flange and all fittings are water-tight.
