Building a questionnaire and taking data from questionnaire to produce a report

    I am wondering does anyone know any free applications or even better any of the Microsoft applications that I can create a survey and then depending on the results populate a report.

    So if I had a generic report "My name is ______ , I go to school in ____ , I have a _____ as a pet.

    If my questionnaire had three questions
    1. What is your name?
    2. What province do you live in (This can fill in a province for where I go school) This can be a multiple choice question.
    3. A question with a checkboxes that lists a load of pets and you can tick which you have.

    If anyone knows anyway I can do this I'd appreciate it.
