Becoming a care representative

Hi. My dad has suddenly ended up in a nursing home and in order to complete the Fair Deal scheme I have to apply to court to become his care representative. I found this CC10 application form on courts.ie, which has four separate forms within. Has anybody any experience of this form and where would I get help filling it in?

I'd suggest you take the forms in to your local Citizens Information Centre, I am constantly amazed at how knowledgeable they are and they are very familiar with the Fair Deal scheme. Good luck.
