Is there either a prescribed form or a recommended form of notice of quitting tenancy used when giving notice to landlord of quitting of a tenancy under the 2004 Act?

Assuming you have a Part 4 tenancy, you just need to inform your Landlord of your intention to leave giving the correct notice based on length of tenancy. It's just a two line letter.

Technically tenants are also required to give notice in writing, the same as landlords. The landlord is unlikely to dispute this with the RTB though.

Here's some sample notices from the RTB, including ones from the tenant to the landlord.
