Do I need am NDLS medical report form for learner permit renewal?

    I'm renewing my Learner Permit for the first time and on the NDLS website it states that I need a fully completed NDLS medical report form, dated within one month, if required in your case.

    How do I know if it's required? Is it only necessary to fill out the form If I have some kind of illness worth mentioning?

    Thanks for any help anyone can give!

    You need a report form if you...
    • Are applying for a learner permit or Driving licence in any of the categories C1, C, D1, D, CE1, CE, DE1 or DE.
    • Will be 70 years of age or more on the first day of the period for which the licence or Learner Permit is being granted.
    • Suffer from certain disabilities or diseases
    • Have ever suffered from alcoholism or epilepsy
    • Are taking, on a regular basis, drugs or medication which would be likely to make you an unsafe driver

    Otherwise you're fine. I think you would know already if you had one of the conditions. You would have needed the form for applying.

    ie, if you answer yes to any of the driver fitness questions on the D201 (with the exception of the corrective lenses question). Just download the D201 and start filling it out.
