Lawn in poor condition

Hi all, this lawn was sown in late spring 2016. Leisure lawn seed I think. Its a big area and it never seems to take hold. From October to may it looks terrible, yellow,thatchy,and now a lot of moss. Hi all, this lawn was sown in late spring 2016. Leisure lawn seed I think. Its a big area and it never seems to take hold. From October to may it looks terrible, yellow,thatchy,and now a lot of moss. In patches it is green and lush but most of it is terrible. I have applied slow release lawn feed in last two springs, I also tested ph levels last spring and they were low around ph5.5-6. Granulated lime was applied last spring. See pics attached. Any advice welcome.
