How to retrieve PUK code from unlocked phone

Bought a phone from Vodafone and have since moved to 3 and recently Virgin. Is it Vodafone i need to contact for said phones PUK?

The PUK code is linked to the sim card, not the phone. So it would be whoever provided the sim card that's in the phone
