Questions on filling out deer hunting licence application form

Q1.When form says minimum 100acres does this mean each permission must be on a farm of minimum 100acres or the total combined permission' s must be a minimum of 100acres.
Q2.when filling out part 2 schedule do you just put the landowners name in column 4.
These might seem like simplistic questions but I want to get application right first time.

100 acres total.

Nope. Name, address, and phone number. Its not a lot of room to be fair so write small.
