renovate or extend kitchen

we're torn on what to do .either way the kitchen has to be done but we're unsure whether or not to extend out ,ideally we would but our garden size is putting me off. is this a situation where we need an architect ? someone who can come in and help steer us one way or the other? and if not an arch then who?
