IIRC IrelandOffline wrote to the C&AG (ComReg's auditors) about this unexploded bomb back in 2016.
ALTO are on record that they won't contribute to a retro-active compensation scheme, and the total amount could be near €190M at this stage.
None of the options look good. Happily no heads will roll and all concerned will move on to better paid jobs.
