If (with a new doctor cert) one needs to continue claiming, do they do a new application or reuse old one?

I presume your original claim is still open and was not terminated by yourself. So your previous doctor should supply your new doctor with all the necessary info to issue the next certs. That should not be a problem because the whole thing is done online now.
