Looks to me like it happened when importing the old message threads from UBB into the new vBulletin system. UBB sticks the code directly into the (static) HTML page of the thread whereas vBulletin generates it all actively on the fly from the database. It just seems that when the import was being done, vBulletin didn't parse out the tags properly and they got left in as text.

It only happens in posts made -before- the changeover anyway.
