This has been a hard watch. Breakdown has been a complete mess. First Carley allows a player to put his hands all over the ball in a ruck, and just warns him. Straight after that, he allows a player to come in around the side of a ruck and pick up the ball.

And all of that almost led to a Toulouse try.
